Why it happens
A flat or low-projection buttock is rarely a gym problem: shape depends on bone structure, fat distribution and genetics. Major weight loss also takes its toll — the buttock loses fat, firmness and position.
Modern surgery offers two routes — your own fat (the Brazilian Butt Lift) and implants — and the choice between them is anatomical, not a catalogue pick.

For slim patients without donor fat: specific implants guaranteeing meaningful augmentation, with the incision hidden in the intergluteal fold.

Before surgery
Your consultation starts with a thorough assessment of your anatomy and goals, followed by a photographic study used to plan the procedure and compare your results later on.
Practical points that apply to every surgery: arrive at a stable weight, have your medication reviewed (antiplatelets and anticoagulants are usually paused beforehand, always together with your doctor) and stop smoking well in advance.
Specific here: the assessment determines donor fat availability — without enough, BBL is not possible and the honest alternative is implants. Tobacco is especially critical for the survival of transferred fat.

Recovery
The peculiarity of buttock surgery is one: sitting. The rest is easier than feared.
Intense muscle-soreness feeling, not pain, thanks to nerve blocks and analgesia at Quirónsalud. Garment on; rest on your front or side.
Avoid prolonged direct sitting for 10–14 days; BBL pillows allow short supported sits without compromising the graft.
Progressive return to routine and work; swelling recedes and the contour starts to define.
The graft has taken: the surviving fat — 70–80% with WAL+EVL — is permanent.

Scars
With fat transfer, the marks are liposuction’s own: 3–4 mm incisions in hidden folds. With implants, the incision sits in the intergluteal fold — naturally concealed, even in swimwear.
Surgical honesty
No biopolymers or dubious ‘acids’ here: your own fat or certified implants. Anything else is a risk no buttock justifies.
Part of the transferred fat is reabsorbed — that is biology. With WAL+EVL 70–80% survives, and that is planned for at infiltration.
Without enough donor fat there is no BBL: the honest alternative is implants. Technique is chosen at the examination, not on Instagram.
The buttock has a muscular component that remains yours. Surgery provides the shape; the tone is your part.

It depends on your anatomy: with spare fat elsewhere, transfer (BBL) wins on naturalness; in very slim patients implants are the honest option for meaningful volume.
Once the graft has taken — around the third month — the surviving fat (70–80% with the WAL+EVL technique) is permanent.
Avoid prolonged direct sitting for the first 10–14 days; BBL pillows help with short sits. After that, progressively back to normal.
