Why it happens

Why is one breast bigger than the other?

No woman has two identical breasts, but when the difference reaches a cup size or more — anisomastia — it stops being an anatomical footnote: it conditions clothes, bikinis, lingerie and confidence. And when the weight difference is marked, the back notices too.

Causes vary: uneven pubertal development, changes after pregnancy and breastfeeding, droop advancing faster on one side, or the aftermath of oncological surgery. Each origin asks for its own solution — and sometimes the ‘normal’ breast is the one operated on.

How two breasts are matched

One-sided augmentation

When one breast stayed small: implant or fat on that side only, chosen to match its partner.

One-sided reduction or lift

When the difference comes from excess or droop on one side: that breast is reduced or lifted.

Lipofilling

Your own fat is symmetry’s best friend: fine adjustments of volume and shape that age like the rest of the body.

Before surgery

Consultation and pre-operative workup

Your consultation starts with a thorough assessment of your anatomy and goals, followed by a photographic study used to plan the procedure and compare your results later on.

Practical points for every breast surgery: arrive at a stable weight, with breastfeeding finished months ago if applicable; have your medication reviewed (blood thinners are usually paused beforehand, with your doctor); and stop smoking. When age or history indicates it, a breast imaging study is completed first.

Specific here: the diagnostic study also allows ruling out hidden pathology — an added health value of this surgery. And symmetry is only planned once breast development is fully complete.

Recovery

Recovery, week by week

The post-op depends on the technique used on each breast, but the general scheme is shared.

First 48–72 hours

Discomfort controlled with analgesia; post-op bra on. If lipofilling was used, donor areas make themselves noticed — normal.

Weeks 1–2

Quiet life and check-ups. Each breast evolves at its own pace — each had its own gesture: temporary differences are expected.

Weeks 3–4

Full routine and progressive exercise when cleared at review.

6–12 months

They depend on each breast’s technique: around the areola or in the fold, placed to stay under underwear lines. When the gesture is lipofilling only, marks shrink to millimetric incisions. Pink at first, discreet with time.

It does not chase 100%

Absolute symmetry exists in no body. The honest medical goal is maximum visual and tactile harmony: a difference that stops being noticed — and stops mattering.

It does not always use implants

Often the best tool is your own fat or a lift/reduction gesture: implants age differently from natural tissue and can unbalance again over the years.

It is not aesthetics only

A marked weight difference between breasts unbalances the back. Matching is also postural health.

It does not operate too early

Full breast development comes first: operating an asymmetry that may still change is betting against time.

Risks and contraindications

Every surgery carries risks and this one is no exception. You will go through them in detail during your consultation and in writing in the informed-consent form; this is the honest summary.

Expected — not a complication

  • Swelling evolving differently on each side the first weeks.
  • Temporary sensation changes, especially if the areola was repositioned.
  • Expectation adjustments: symmetry is judged once tissues settle.

Uncommon

  • Uneven evolution over the years needing an adjustment.
  • Partial fat reabsorption.
  • More visible scarring than expected.

Breastfeeding: preserved in most symmetrisations, though deep glandular reduction can affect it. If it matters to you, condition the plan at consultation.

Is one breast operated on, or both?

At what age can asymmetry be corrected?

Once breast development is fully complete. Operating earlier, on an anatomy that may still change, risks the asymmetry returning.

Will the result last as I age?
