Why it happens
More and more men come to the clinic after major weight loss — with Ozempic, Wegovy or Mounjaro, with diet or after bariatric surgery — with a problem exercise cannot fix: surplus skin. Skin stretched for years does not go back when the fat disappears.
In men there is another factor: male fat is mostly visceral (inside the abdomen), not under the skin. That fat cannot be suctioned; it only goes down with weight. That is why the surgical plan is decided once the weight is stable.
A tummy tuck solves what is surgical: it removes the surplus skin, repairs the diastasis and redefines the waist without narrowing it: a male abdomen should be flat and straight, not hourglass-shaped.

When the excess skin is limited to the area below the navel: shorter scar, no navel repositioning and a quicker recovery.
Love handles are not solved by the tummy tuck alone. Male liposuction of the flanks in the same operation defines the contour while respecting the proportions of the trunk.
After massive weight loss, with surplus skin horizontally too: a vertical incision is added to the low one. More scar, but the only way to tighten the whole abdomen.
Am I a candidate?
The clearest indication: the weight has already come down and stabilised, and the skin hangs over the belt.
An abdomen that bulges even without fat, after years of overweight. Muscle plication corrects it.
With the weight still dropping or a high BMI it is better not to operate: the result would be lost and the risk goes up. Weight first, surgery after.
Visceral fat (the "hard belly") cannot be operated: it depends on weight. If that is the case, the honest thing is to say so in consultation.
Aftercare
First week: one night at Quirónsalud Málaga Hospital, drains for a few days and a slightly flexed posture when walking and sleeping. Discomfort controlled with the prescribed medication.
Weeks 2 to 4: abdominal garment day and night; two to three weeks off work depending on the job. Walking from day one, which also lowers the risk of thrombosis.
From week six: progressive sport with the doctor's approval; abdominal exercises and heavy loads a little later. The scar matures and fades over the first year; strict sun protection meanwhile.

When not to operate. Not with active smoking: it must be stopped one or two months before. Nor with unstable weight or a high BMI, nor if the belly is visceral: that calls for a different answer.

The technique is the same, but the goal and the design change: in men the waist is not narrowed, the scar sits lower and straighter and the plication aims for a flat, not concave, abdomen.
It is the most frequent case today. The condition is that the weight has been stable for at least three to six months. In consultation we assess whether a full tummy tuck is enough or the fleur-de-lis version is needed.
